You must complete the England Hockey Player Assessment

England Hockey have announced we have moved to Step 4 which allows us to tackle and play internal/local friendlies. In order to move to this step, there are a number of important administrative tasks both the club and participants (including players, officials and coaches) need to take.

1. In order to play, you must complete the England Hockey Player Assessment (it only needs to be filled in once). You will not be able to play if you have not completed the form at least 24 hours before you play your next session (sessions on Monday 10th August will be as per step 2 guidelines). The form is found by following the link or can be found on the England Hockey Covid19 Club Support page.

2. All attendees (including coaches, parents/guardians and officials) must read the attached Stage 4 Club guidelines before they attend their next session

3. All attendees must undertake a Covid19 self-assessment before they attend any match or training session. This is a personal check that you or your household are not displaying any Covid 19 symptoms and have not been in contact with anyone who has or is suspected of having Covid 19. If you do not meet these requirements, you must not attend and must let the Club Covid 19 officer know. In attending the session, you are confirming that you have completed your self assessment.

4. Bookings for Summer Hockey sessions will continue to be made via our weekly form, with payment via Pitchero. All those attending ladies and mens 1/2s pre-season training MUST confirm their attendance via the Pitchero app.

5. All players must report to the session admin to register attendance upon arrival

The CYHC Covid 19 officer is Lily Rowntree. Lily can be contacted via email at fixsec@cityofyorkhc.co.uk or via pitchero messenger. Please contact Lily immediately if you have attended one of our sessions (including parents/guardians etc) and are showing symptoms of Covid19 or have been in contact with someone who has tested positive for, or is suspected of having Covid19.
